On this page you will find the CLIENT REGISTRATION FORM and the NEW PATIENT FORM.

 

Please, take a moment to fill them out as completely as possible.The information you provide is important and will help us serve you and your Pet better and expedite the registration process.This requirement is one-time only. We ask you to submit them prior to the appointment.

 

These are online Forms which can be filled in from your computer using the free Adobe Acrobat Reader. When the forms are complete, print or save them and send to us. You can also choose to print the Forms and complete by hand. If you prefer, call us to have the blank forms e-mailed or mailed directly to you.

One of the common species brought in for rehabilitation, usually as babies. They are cared for and released when they are in good condition.

This little girl was brought to SCWC with a complicated fracture of the right hind leg.

This huge Snapper had a fishing hook stuck in her esophagus and part of the line in her stomach. Both were safely removed and she was released back into the wild.

These turtles were waiting to be X-rayed to screen for foreign bodies and presence of eggs. SCWC has a small veterinary unit to provide medical treatment for incoming patients.

Moritz was found injured and brought to SCWC. Because it was a domestic goose, after recovery it was taken to Poplar Spring Animal Sanctuary for release.
